چکیده
1. A contrast-enhanced aortic MDCT demonstrated four localized dissections of the descending and the abdominal aorta (Panel A; reconstructed coronal view to show al four dissections). Panels B to E are axial views of each aneurysm. The first segment was at the top of the aortic arch (Panel B), the second was located in the retro cardiac aorta (Panel C), the third was at the level of the diaphragm (Panel D), and the fourth was located below the renal arteries (Panel E). The largest was saccular in shape and 5-cm in diameter (Panel C). □ PICTURES IN CLINICAL MEDICINE □

Type A Aortic Dissection Presenting with Acute Lower Extremity Vascular Insufficiency in the Absence of Chest Pain
We report a case of a 53 year-old man with a history of hypertension presenting with acute left lower extremity parasthesias and pulselessness initially presumed to be secondary to arterial thrombosis or embolism. Work-up included a transthoracic echocardiogram which revealed an aortic dissection at the level of the aortic root extending to the visualized portions of the descending aorta. Type ...
متن کاملPainless dissections of the aorta presenting as acute neurologic syndromes.
We describe three patients who had painless dissections of the aorta which resulted in neurologic syndromes at the time of presentation. Two patients had acute hemimotor and sensory findings. In one of these cases progression to paraplegia occurred. In a third patient, acute weakness and ischemia of a leg occurred at presentation. We review previously described painless aortic dissections. Such...
متن کاملUnexpected cause of a right hemiplegia secondary to the painless full-length aortic dissection: a case report and literature review.
Painless aortic dissections in general are uncommon and are frequently misdiagnosed. Here we reported a rare case of acute ischemic stroke secondary to completely painless acute full-length dissection (DeBakey I) and provide a brief review of the literature. A 56-year-old man was referred to our department with right hemiplegia. Ischaemic stroke and thrombolytic treatment were considered initia...

rare malady and it has been only infrequently reported on; its typical course and optimal management are not clearly defined (1). It is usually limited to the infrarenal aorta (1, 2). Isolated abdominal aortic dissection may be classified on the basis of its etiology as iatrogenic, traumatic or spontaneous (2). Primary abdominal aortic dissections comprise less than 2% of all aortic dissections...
متن کاملInter-arm and Inter-leg Systolic Blood Pressure Differences in Aortic Dissection
The essential feature of an aortic dissection is a tear in the intimal layer of the aorta, followed by formation and propagation of a subintimal hematoma. It is a medical emergency with a high mortality rate if appropriate management is not instituted immediately. There are two different systems classifying the aortic dissections. The Stanford classification divides dissections into two types: ...
